Provide specialized care to specific patient populations, ensuring optimal care and coordination between providers, patients, and healthcare teams. Act as a liaison for patient testing, scheduling, and referrals while assisting in patient education and the implementation of care plans.
MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Current unencumbered licensure with the WV Board of Licensed Practical Nurses, or the appropriate state board where services will be provided, as a Licensed Practical Nurse. 2. State criminal background check and Federal (if applicable), as required for regulated areas. 3. Certification in Basic Life Support within 30 days of hire. EXPERIENCE: 1. Five (5) years' experience as an LPN. PREFERRED QUALIFICATIONS: EXPERIENCE: 1. Specialized LPN experience. CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. Provide hands-on assistance to patients in accordance with the care plan, including preparing or administering medications, performing basic tests, assisting with procedures, and obtaining specimens under the supervision of an RN or physician. 2. Act as a point of contact for patients, coordinating appointments, testing, and procedures as needed. Work with scheduling departments to ensure timely care. 3. Support care plan implementation by assisting in the implementation of care plans developed by RNs and physicians, ensuring appropriate follow-up and delivery of care as per the established plan. 4. Assist with obtaining prior authorizations for patient services, working with healthcare providers and insurance companies as needed. 5. Provide basic education to patients about their conditions and treatments under the direction of an RN or healthcare provider. 6. Accurately documents patient care activities, assessments, and progress according to departmental guidelines and under the supervision of an RN or physician. 7. Work with RNs, physicians, and other healthcare providers to ensure the continuity and quality of patient care. Assist in administrative tasks as necessary to support patient care delivery.
