The IKC Care Coordinator Coach is an experienced professional and subject matter expert who supports the IKC Care Coordinator team. This role is focused on enhancing the skills and effectiveness of Care Coordinators through targeted coaching, education, and process reinforcement. The CC Coach acts as a mentor and a resource, ensuring the team delivers high-quality, consistent, non-licensed clinical support to patients and successfully integrates with the broader care team.

Essential Duties and Responsibilities

The Care Coordinator Coach's work is centered on elevating the performance and capabilities of the Care Coordinator team.

Coaching and Mentorship

  • Provides one-on-one and group coaching to Care Coordinators to improve performance in patient engagement, care coordination, and team collaboration.

  • Serves as a primary mentor for new and existing Care Coordinators, guiding them through onboarding and ongoing professional development.

  • Conducts regular quality reviews (e.g., call audits, documentation checks) and provides constructive, respectful feedback to teammates.

  • Assists Care Coordinators in managing complex patient cases or challenging situations by providing expert guidance and de-escalation support.

  • Acts as a role model, demonstrating best practices in all aspects of the Care Coordinator position.

Education and Reinforcement

  • Develops, updates, and delivers training materials and job aids on workflows, software applications (e.g., Cerner, CXone), and soft skills.

  • Leads training sessions for new hires and facilitates ongoing education for the team on new processes or program updates.

  • Identifies knowledge gaps and skill development opportunities within the team and collaborates with leadership to develop targeted training initiatives.

Subject Matter Expertise and Process Improvement

  • Functions as the go-to subject matter expert (SME) for all Care Coordinator workflows, policies, procedures, and relevant technologies.

  • Participates in the development and refinement of IKC Model of Care clinical coordinator processes, providing valuable feedback from a front-line perspective to improve efficiency and patient experience.

  • Assists with the rollout and implementation of new tools and workflows, ensuring smooth adoption by the Care Coordinator team by providing peer-to-peer support.

  • Analyze team performance data and reports to identify trends and opportunities for process improvement.

Collaboration and Support

  • Collaborates closely with Managers and Supervisors to support team goals, projects, and performance objectives.

  • May be required to cover Care Coordinator duties as needed to support the team during periods of high volume or short staffing.

  • Participates in care team huddles and other meetings to reinforce best practices and ensure alignment with clinical partners (NPs, RNs).

Qualifications

  • High school diploma or equivalent required; Associate's or Bachelor's degree preferred.

  • Significant experience and demonstrated high performance in a Care Coordinator or similar patient-facing healthcare coordination role.

  • Proven experience in training, mentoring, or coaching peers is highly desirable.

  • Expert-level proficiency in IKC workflows, documentation tools (e.g., Cerner, CXone), and internal reporting.

  • Exceptional communication, interpersonal, and active listening skills.

  • Strong ability to provide constructive feedback in a positive and professional manner.

  • Excellent organizational, problem-solving, and critical-thinking abilities.

  • Demonstrated ability to work independently and as part of a collaborative team

  • Prior IKC Experience

The Wage Range for the role is $30.50 - $38.00/hour.
