As a Mental Health Rehabilitation Specialist, you will support older adults (60+) in navigating the complexities of mental health, medical care, and housing. Whether you are working in the community to prevent institutionalization or helping conserved members transition to the "least restrictive environment," you are the ultimate champion for their independence.

  The Clinical Advocate: You'll provide direct community support and case management using Psychiatric Rehabilitation principles and Recovery Philosophy.

  Systems Navigator: You'll be the bridge between our members and critical agencies, collaborating with the Public Defender's Office, Conservator's Office, and Protective Services to ensure your members are protected and heard.

  Crisis Interventionist: You'll provide the steady hand and trauma-informed care needed during difficult moments, ensuring our members feel safe and supported.

  Whole-Person Care: You won't just manage a "case"-you'll coordinate medical, psychiatric, and natural supports to ensure a comprehensive path to wellness.

  
  

Schedule: Monday - Friday | 8:00am to 4:30pm | Full-Time

Expected starting wage range is $26.50 - $32.50. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• Master's degree in Social Work, Psychology, Counseling, Human Services or Social Services and two (2) years of experience providing direct services in Mental Health; or Bachelor's degree in Social Work, Psychology, Counseling, Human Services or Social Services and four (4) years of experience providing direct services in Mental Health; or an Associate's Degree and six (6) years of experience providing direct services in Mental Health
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to attend meetings, drive clients to appointments and groups, etc.
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.

AgeWise is a community-based program serving older adults. AgeWise serves a maximum of 600 individuals, aged 59+. Agewise's Strengths-Based Case Management is a consumer-driven and highly collaborative program. Teams work with members to identify needs and build a treatment plan that is collaborative and comprehensive. The goal is to assist people living in the community and ensure members are linked to housing, medical care, psychiatric care and natural supports. Agewise's Institutional Case Management (ICM) serves members who are conserved and placed in a long-term care facility. The program provides members with support in obtaining and maintaining benefits, care coordination, discharge planning, and advocacy. Members are assessed to ensure that they are placed in the least restrictive environment possible.
