This role within Access to Care's Medication Assistance Team primarily supports patients undergoing active treatment for a cancer diagnosis at an MHMG Cancer Care location. The position manages a caseload and collaborates closely with oncology practices,pharmacistsand social work to help patients obtain needed medications through grant funding, copayassistance, or free medication programs offered by pharmaceutical companies. In addition, the roleassistsinidentifyingand connecting patients to other supportive resources, such as Medicaid and the Medicare Savings Program.

Summary:
The Case Manager - Medical Outreach role is responsible for the provision of case management services and achievement of positive intervention outcomes for assigned clients. They work on-going with patient caseloads, delving deeper into more medically and socially challenging issues and health needs, addressing barriers for patients.

Required Minimum Knowledge, Skills, and Abilities (KSAs)


  1. Education:Bachelor's Degree preferred, with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, Nursing, or closely related field preferred.
  2. License/Certifications:Current applicable state(s) license as a Licensed Social Worker preferred. Valid driver's license and reliable vehiclerequired.
  3. Experience:With a bachelor's degree: Two years of experience in a community health setting, including one year of case management experience preferred. In lieu ofbachelor'sdegree: Three years of directly relevant experience in a healthcare setting, including 6 months of case management experience. Experience collaborating with state and federal agencies, community agencies, municipal, and public programs preferred.
