Position Summary:
The FGP Medical Records Clerk will perform routine clerical work relating to the maintenance of medical records. In this role, the Medical Records Clerk will be responsible for managing patients health records and history, gathering and filing patient information, and scanning all medical record related items in the EMR system. The FGP Medical Records Clerk will ensure that all health information is maintained in accordance with established NYU Medical Center policies and procedures, and applicable regulatory standards, as well as regional, state and federal guidelines.
Job Responsibilities:
* Scans all medical record related items in EPIC following NYULH and FGP scanning guidelines for how to scan, notate and link information correctly in the EMR to facilitate ease of locating information. This information includes outside NYU (non-interfaced) test results, medical clearances forms, INS forms, consent forms, letters and consult notes from outside NYU providers and other clinical information received via Fax, paper &/or electronic form into the appropriate chart and location.
* Releases medical records in accordance with HIPPA and FGP ROI guidelines. Documents disclosures in EPIC and fulfills requests within a 10 day timeframe.
* Ensures appropriate NYULMC medical records release form is signed and dated in accordance with request.
* Responsible for safeguarding patient records and ensuring compliance with HIPAA standards.
* Ensures patient charts, paperwork and reports are completed in an accurate and timely manner.
* Issues medical files to persons and agencies according to laws and regulations to ensure confidentiality and integrity utilizing the release of information functions in EPIC within 10-day timeframe.
* Collects and processes payments received for fees billed for medical records requests within EPIC when appropriate
* Reviews authorizations for release of information's from appropriate parties when needed.
* Assists in-person or via phone medical records requestors. Answers the telephone in a polite manner, identifying themselves and the department. Takes messages thoroughly and accurately. Answers calls promptly and gives out accurate information to routine questions. Transfers calls to the correct individual based on content of inquiry.
* Communicates with physicians regarding chart review prior to release of records.
* Escalates all legal subpoenas to appropriate leadership
* Ensures receipt of all records and documents in accordance with departmental procedures and policies. Follows-up on any missing documentation.
* Contacts NYULMC merge center regarding duplicate medical record numbers.
* Acts as a liaison to assist with questions related to appropriate patient records/documentation;
* Assists with medical record transferred and releases from merging medical practices.
* Ensures patient historical medical records documentation are uploaded into EPIC in a timely manner.
* Follows scanning guidelines and best practices for importing outside documents-results, records, forms, etc.
* Remains current on latest best practices, policies, and protocols (including workflow updates, FOCUS trainings, job-related tasks);
* Participates and performs all other related duties.
* Works collaboratively with colleagues and site management to ensure a positive experience and timely resolution for all patient interactions and inquiries whether in person, by phone or via electronic messaging.
*Partners with Patient Access Center and Central Billing Office team members to support collaboration and promote a positive patient experience.
* Performs other duties as assigned.
Minimum Qualifications:
High School Diploma or the equivalent required. Good communication, attention to detail and computer skills required. Typing skills and familiarity with automated systems and PC applications preferred. Ability to complete multiple tasks efficiently and thrive in a team work environment which pursues a positive patient care experience.
Preferred Qualifications:
Bilingual.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $50,468.99 - $50,939.53 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
