Essential Functions: The Oncology Care Transition Coordinator is a registered professional nurse who under the supervision of the Oncology Care Transitions Manager contributes to the care of any hospitalized cancer patients by engaging patients in the transitional care management process and by collaborating with the inpatient and ambulatory clinicians. This person meets with patients upon admission to the inpatient unit, participates in multidisciplinary daily rounds, ensures the coordination and confirmation of appointments and follow up requirements.
