What You Will Do to Change Lives

You'll work directly in the Central Orange County community, providing life-saving medical support and psychiatric care to individuals transitioning out of homelessness. As an LVN or LPT at Telecare TAO Central, you aren't just passing meds-you are a cornerstone of recovery. You will:




  • Deliver recovery-focused services and medication support in a way that respects the dignity of every member.



  • Work as part of a multidisciplinary team to address complex health and housing needs.



  • Be the bridge between clinical care and real-world stability for our members.




Shifts Available: Full-Time | 8:00 AM - 4:30 PM | Monday - Friday

Expected starting wage range is $28.50 - $35.00.Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)



  • Current/Active Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT)
  • Current CPR/BLS, First Aid, and CPI certified (hands-on course)
  • San Bernardino County FACT programs: Must have two (2) years of experience working with individuals with serious and persistent mental illness
  • Must be at least 18 years of age
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal or company vehicle to drive clients to appointments and groups, etc.(weekly mileage reimbursement at the IRS rate)


All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver license, a motor vehicle clearance, and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual, or local requirements may apply.

The program will emphasize enhanced engagement and outreach efforts along with intensive case management, Vocational/Educational/Employment support and robust housing strategy services.
