The Social Work Aide III provides age appropriate assessments, counseling and social services to clients across the life span. This includes enhancing the conditions of the client by providing information and education; making referrals and when necessary, making home visits and follow-ups. In addition, this position acts to help clients, as a mediator, and to utilize and obtain needed services at Mile Square Health Center. The unpredictability that can occur from day to day requires the individual to be able to adapt rapidly to change by adjusting work schedules, activities, and the proper allocation of resources.

Duties & Responsibilities:
  • Provide social services to clients of all ages, including assessments, case planning, follow, up and home visits when deemed necessary.
  • Prepare care plans describing the services the client may need, use, and the method of assessing medical and/or drug treatment and needed social services.
  • Advocate on the clients' behalf to obtain necessary and appropriate services; provides referrals and follow up services within a multidisciplinary team of providers.
  • Provide patients with information and resources and assist with support services and; such as disabilities (SSI), food pantries, guardianship, Meals on Wheels, homeless shelters/housing, childcare assistance programs, homemaker services, home health, services through the Department of Aging, parenting classes, transportation, mental health issues, substance and domestic abuse counseling, etc.
  • Assist patients in the health market place enrollment process requiring certification as a Certified Application Counselor.
  • Follows up or directs the Social Work Aide I to ensure that services were assessed and are appropriate to the client's needs.
  • Assembles and maintains necessary statistical information for administration as required i.e. daily work logs
  • Maintain complete and accurate files on all client programs
  • Participate in agency on-call system, team staffing, and local coordinating committee meetings.
  • Represent Mile Square at community events during regular and off hours.
  • Perform other related duties and participate in special projects as assigned.


Minimum Qualifications Required:

1. Two years of work experience comparable to a Social Work Aide II

2. Any combination totaling two years from the following categories:

A. work experience which provided knowledge of human behavior, including the ability to cope with a variety of people and/or understand the importance of people's needs and feelings, in a public/customer service environment.

B. college course work in social or behavioral sciences or related field, measured by the following conversion table or its proportional equivalent:
  • 15 semester hours equal six months
  • 30 semester hours equal one year

Preferred Qualifications:
  • Ability to work with myriad of patients from various racial, ethnic and socioeconomic backgrounds.
  • Ability to motivate staff to achieve desired results.
  • Ability to work independently, exercise good judgment, and make sound decisions.
  • Ability to establish and implement policies and apply standards consistently.
  • Must possess excellent interpersonal skills in order to establish and maintain professional relations with other members of the health care team.
  • Ability to communicate effectively both orally and in writing.
  • Ability to translate (English/Spanish) for patients and others as necessary a plus
