About the Job

Responsibilities
HIV Testing and Prevention (60%)
HIV Testing:
Provide CLIA-waived HIV testing services at office, community, mobile, and outreach
locations
Mail, deliver, or distribute test kits for at-home self-testing, then conduct HIV testing
sessions (in person or via video conferencing) upon request
Complete the HIV Risk and Health Assessment with clients during testing
Provide education and referrals based on client needs with all clients
Provide education and follow-up via Zoom or video conferencing, when necessary
Follow all state and CDC requirements, including reporting reactive tests to the
Minnesota Department of Health

PrEP and PEP Coordination (20%)
Provide PrEP and PEP education during testing and outreach with all clients
Identify clients eligible for PrEP or PEP and assist with appointment scheduling when
possible
Refer clients for STI testing, treatment, and syringe service programs when indicated
Conduct warm handoffs to partner providers when possible and follow up to confirm
access
Troubleshoot access barriers and provide follow-up support

Linkage to HIV Care:
Call HIV clinics with the client present to schedule same-day or next-day appointments
Ensure all clients with a reactive test are linked to HIV medical care within 7 days
Follow up within one week to confirm attendance and address barriers
Connect clients to case management, mental health, housing, food, and harm reduction
services based on identified needs
Maintain ongoing support as appropriate with all clients who need additional services
Relationship Management and Outreach-
Participate in coordinating and conducting outreach events, maintaining outreach and
HIV testing calendar and communication
Provide outreach at scheduled events, offering testing access
Provide education and follow-up via Zoom or video conferencing, when necessary

Administrative Tasks (10%)
Program Administration:
Document outreach contacts, referrals, and follow-up outcomes with all clients
Collect required program data for monthly and quarterly tracking; Assist with quarterly
reports to funders
Participate in organization-wide strategic planning
Participate in weekly staff consultations and supervision

Community Engagement and Health Professions Learner Education (5%)
Community Development:
Identify and build relationships with individuals and agencies that could benefit from YAP
services (and vice versa)
Collaborate with community agencies with similar grants/goals to promote coordination
Plan and coordinate (with YAP staff and community partners) participation in large-scale
events such as Youth Pride, Q-Quest, Queer Prom, and the All-Provider Meeting

Professional Development and Continuing Education (5%)
Attend HIV trainings and conferences as required, recommended, and available
Maintain knowledge of resources available to target populations
Maintain training in HIV testing/counseling best practices and motivational interviewing
strategies

Qualifications

Required Qualifications
Bachelor's degree in social science or health-related field OR Four years of related experience

Pay and Benefits

Pay Range: $24.50 per hour depending on education/qualifications/experience.
