SUMMARY: The Program Coordinator under general direction reports to the Director of Community Health Institute. The Program Coordinator plays a key role in integrating Connect for Health into clinical services, working alongside health care providers to develop efficient systems for resource connection, managing and coaching undergraduate Advocates, using data to enhance Advocate performance, and assuring the quality of services to clients.   • Oversee, develop, and motivate a team of approximately 60 undergraduate student Advocates from Brown University
  • Provide daily support for Advocates at the clinic and at various on campus meetings to guide professional and leadership development
  • Develop and manage a team of student leaders who oversee and support peers working as Advocates through weekly meetings, individual coaching sessions, and trainings
  • Plan and lead weekend-long trainings for new Advocates and for Student Leaders each semester
  • Coordinate the delivery of weekly Advocate Development sessions to Advocate Corps along with Brown University's Swearer Center
  • Observe and evaluate Advocates' performance at clinics and on campus as well as keep abreast of the most complex cases
  • Monitor and facilitate Advocates' communication with healthcare providers
  • Ensure Advocates' compliance with Connect for Health performance, attendance, and reporting standards
  • Undertake direct casework with a limited caseload during semester as needed, and during breaks in the academic calendar to ensure continuity of client services
  • Communicate innovations and challenges to director and appropriate clinical colleagues
  • Create trusting, productive, and responsive relationships with physicians, nurses, referring providers, and other clinic staff
  • Support Connect for Health integration into the clinical setting through strategies such as implementation of screening and referral systems, and ongoing training for clinic staff to screen for social determinants of health and refer patients to the Connect for Health desk
  • Participate as a full member of the clinic team, including attending team meetings and acting as an advocate for Connect for Health
  • Build and maintain a local network of resource providers, including site visits and presentations to educate agency staff about Connect for Health and create streamlined referral processes
  • Collaborate with director and clinical team to establish quarterly program targets
  • Design and implement standards for quality case management and quality improvement
  • Ensure the accuracy of programmatic outcomes as reported in the client management database
  • Continually monitor outcomes and report data to director on at least a monthly basis
  • Use program data to manage towards program milestones, related metrics and improve Connect for Health operations
  • Collaborate with the social work and clinical teams to dynamically assess program design and strategy
  • Oversee Advocate completion of required hospital volunteer enrollment process
  • Manage recruitment, selection, and onboarding of paid Advocates to provide client services during semester gap periods
  • Troubleshoot logistical concerns brought by Advocates and designated hospital staff such as problems with client management database, scheduling, and supply needs
  • Manage Program budget- track and order supplies for desk operations, campus trainings and event needs
  • Coordinate annual Advocate graduation event
BASIC KNOWLEDGE:
  • Bachelor's degree and 3-5 years of experience in case management and/or client services with a low-income population required; advanced degree in related field desirable (LICSW or LMHC strongly preferred)
  • Bilingual (Spanish/English) strongly preferred
EXPERIENCE:
  • At least two years of program management experience required and previous experience with program start-up desirable. Previous experience in a hospital setting is desirable
  • Demonstrated experience successfully leading and managing a team of young staff or volunteers and leading interactive group training sessions
  • Strong track-record working with community agencies and/or performing case management in clinic or other setting
  • Proven knowledge of landscape of public benefits and community resources and how to facilitate individuals' access to those resources
  • Excellent interpersonal and communication skills, with the ability to engage and work closely with a wide range of constituents including the Connect for Health clients, college undergraduates, health care providers, and health system managers
  • Strong organizational, time management and self-management skills, outstanding attention to detail, and high degree of flexibility and self-direction
  • Comfort and acumen with data analytics, quantitative and qualitative data, and persuasive presentation skills
  • Grant writing experience preferred
SUPERVISORY RESPONSIBILITY: Oversight of 60+ student volunteers

Pay Range:

$53,060.80-$87,547.20
