Mercyhealth, a community-based healthcare network serving southern Wisconsin and northern Illinois, is seeking a Cardiology Nurse Practitioner or Physician Assistant to join our growing team at Mercyhealth Specialty Care Clinic - Stephenson St. & Mercyhealth Memorial Hospital in Freeport, IL.

Opportunity:

  • Provide comprehensive cardiology care in both outpatient and inpatient settings, offering continuity for patients across the care spectrum
  • Full-time position with a highly desirable schedule, no weekend call; 1st call only two nights per week
  • Manage a patient panel in a collaborative practice environment with autonomy and immediate physician support available
  • Work in a well-equipped clinic featuring 8 exam rooms, a dedicated pacemaker room, and on-site diagnostics including EKGs and Dopplers
  • Join a department actively growing to meet the strong demand for cardiology services in the community
  • Supported by a robust care team including RNs, LPNs, Medical Assistants, and an Operations Team Lead

Responsibilities:

  • Provide high-quality patient care for a predominantly adult and geriatric population (approximately 75% Medicare)
  • Manage an average of 16 office patients per day, performing assessments, follow-ups, and chronic disease management
  • Assist with pacemaker evaluations and supervise cardiac stress tests
  • Champion patient education on heart failure management, lifestyle modifications, and medication adherence to reduce hospital readmissions
  • Participate in triage and workflow processes to ensure patients are directed to the appropriate provider
  • Collaborate closely with physicians and interdisciplinary teams to ensure coordinated, patient-centered care

Qualifications:

  • Current Nurse Practitioner (NP) or Physician Assistant (PA) licensure in Illinois (or eligibility)
  • BLS and ACLS certifications required
  • Experience in Cardiology preferred (or strong desire to specialize)
  • Ability to work effectively in a team-based care model with excellent communication skills
