The Registered Nurse Clinical Educator specializes in diabetes care, provides expert guidance and education to patients, families, and interdisciplinary teams on effective diabetes management. The educator designs and delivers evidence-based training programs that promote self-care, lifestyle modification, and glycemic control. Through collaboration and clinical leadership, the RN diabetes educator supports improved patient outcomes and fosters a culture of continuous quality improvement and learning in diabetes care. The diabetes nurse educator serves to assess, assist, and educate diabetic patients in partnership with the provider. This includes: -Instruct patients on integrating diabetic medications with lifestyle, in order to achieve optimal diabetic control. -Educate patients on complications of diabetes. -Assist with psycho-social needs associated with diabetic management. -Assist patients with utilization of latest technologies in the ongoing treatment of diabetes. -Participate in individual and group education courses. -Acts as a mentor and resource to staff. Assist with the education of clinical staff and provide clinical input related to completing MD and patient requests. -Develop programs for education of providers, work on referral and community outreach, explore opportunities to work collaboratively with other centers, assist with patient clinical research as needed. Essential Duties:
- Patient Diabetic Education and Counseling: a. Participate as an integral member of the diabetes care team which includes the patient, family, physician, and Keck Medicine staff. b. Instruct patients on integrating diabetic medications, both oral medications and intense insulin management, for both type I and type II diabetic patients. Help promote healthy lifestyle changes such as meal plans and increased activity on a daily basis. Guide patients in their personal goals and help them formulate an action plan to improve their diabetes self-management. Help patients interpret their blood glucose data and changes that may need to be made by their endocrinologists. c. Educate patient on the complications of diabetes and how to minimize occurrence and impact. d. Assist patients with the psycho-social adjustments associated with a diabetes diagnosis. e. Assist patients with the utilization of the most modern technologies associated with diabetes management. f. Provide insulin pump training. g. Provide training in the utilization of glucose sensors. h. Provide group diabetic education. i. Schedule follow up with patients and their families whenever necessary. j. Be available to patients and their family via scheduled appointments and by telephoneor patient portal. k. Research and stay abreast of nutrition and diabetes guidelines. l. Help coordinate communication amongst endocrinologists and primary care physicians.
- Documentation: a. Document the diabetic education and counseling in the patient's medical charts. b. Provide the patient or family with a written plan to guide them at home. c. Document attendance at diabetes support group and diabetes education class.
- Administration: a. Propose medications to provider and plan of care changes to providers via electronic health record. b. Maintain communication with patients through patient portal c. Assist with patient scheduling in EHR
- Education Program Coordination: a. Participate in diabetes education team. b. Market and promote the program working with public relations & marketing, and other outside resources. c. Telehealth and DPP for community and digital education programs d. Present and develop group classes relative to diabetic care, basic insulin injection instruction, and medication management.
- Additional Diabetes Care Team Responsibilities: a. Develop diabetes educational materials for patients and classes. b. Assist Director of Diabetes in coordination, recruitment, data collection for clinical research within diabetic patient population. c. Stay current with American Diabetes Recommendations as well as other cutting edge therapies in both nutrition, diabetes management, and treatment. d. MMaintain A.D.A. accreditation through data collection and reporting. e. Educate, train and coordinate research projects with nurses, research coordinators, and others in the care of diabetic patients.
- Performs other duties as assigned.
Required Qualifications:
- Req Bachelor's Degree Degree in Nursing
- Req 2 years Minimum two (2) years formal or informal experience in educator role.
- Req 2 years Experience in diabetes management or endocrinology.
- Req 3 years Experience as a Registered Nurse.
- Req Proficient in use of computer and applications such as Excel, Word, PowerPoint, internet access and utilization
- Req Proficient in verbal, written, and graphic presentations.
- Req Ability to work independently with minimal direction and supervision.
- Req Ability to effectively work with physicians, staff, and patients.
- Req Ability to utilize effective, appropriate and diplomatic oral and written communication skills.
Preferred Qualifications:
- Pref Master's degree Master's Degree in Nursing or related field, or in progress, strongly preferred * Master's Degree in Nursing may be in lieu-of Bachelor's Degree in Nursing
- Pref Experience in Clinical Research.
- Pref ADA program coordination and recognition preferred
Required Licenses/Certifications:
- Req Registered Nurse - RN (CA Board of Registered Nursing)
- Req Basic Life Support (BLS) Healthcare Provider from American Heart Association
- Req Fire Life Safety Training (LA City) If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only)
- Req Certified Diabetes Care and Education Specialist - (CDCES) Certified Diabetes Care and Education Specialist (CDCES), If no card upon hire, one must be obtained within six months of hire.
The hourly rate range for this position is $58.50 - $96.53.
