The Pharmacy Technician Medication Access is primarily responsible for coordination of medication access for patients and families. This position also spends a portion of time performing the job duties of a Pharmacy Technician; under the direction of a licensed pharmacist and following established policies and procedures, performs a variety of technical procedures to support hospital inpatient or outpatient pharmacy operations. This position will help support the expansion of the Transitional Care Management (TCM) Program. The TCM Program is an evidence-based care coordination intervention focused on the first 30 days following hospital discharge-a critical period for patients with medical complexity. This position will work in partnership with a Family Health Navigator to enhance care coordination for children with medical challenges. The employee in this role will assist in timely medication access, identify potential issues, and provide support to families and care teams during the transition period. Additional Information This position is made possible by and is funded through third party grants and/or restricted funding. The length of the assignment is dependent on the grant program and on funding availability. Based on current funding, the anticipated end date for this assignment is 9/30/2026. This position will be reviewed and may be extended in accordance with the grant. Should this position be discontinued and/or eliminated for any reason, it is not eligible for severance.
