Role Summary As an essential member of the Chicago Department of Public Health's Health Protection Bureau, Program Operations Division, the Community Resource Navigator facilitates linkages to lead poisoning testing for children in Chicago and referrals to critical health services, including primary care, nutrition services, developmental screenings, and other care management needs for children with elevated blood lead levels. Under the supervision of the Assistant Commissioner for Health Protection, this position will also serve as a key liaison to CDPH lead abatement program, Federally Qualified Health Centers, free and charitable clinics, community-based organizations, and other safety net providers. Additionally, the position supports community outreach and education efforts related to food safety. ESSENTIAL DUTIES * Assists clients in accessing programs and resources (e.g., medical care, housing, education, public assistance and other support services) to enhance their physical, mental and social health * Conducts home visits to gather data related to health and environmental conditions * Utilizes scheduling platforms, email, and telephone to communicate with clients and service providers * Follows up with clients, clinicians and other service providers to ensure timely coordination of services, addressing inquiries, scheduling appointments, and providing updates or necessary information to support client needs and service delivery * Enters and updates client information into databases to ensure accurate and up to date records * Tracks client referrals and interactions within the system * Assists clients in completing forms to apply for applicable health and social service assistance and programs * Researches and identifies current and available community resources * Coordinates the provision of services with departmental staff, local health and social service agencies and community groups * Provides information on the program to community groups and agencies * Maintains lists of health and social service providers in the community * Performs outreach work by making presentations at churches, community groups, health fairs, organizations, etc. * Coordinates work efforts with community organizations and social service agencies to ensure support services and assistance are being offered and provided to non-English speaking clients * Conducts support and group trainings to clients on health and wellbeing practices * Maintains electronic records including client files, forms adhere with confidentiality and data protection standards and prepares work activity reports Performs other related duties as assigned. Two (2) years of community or social service experience, OR * An Associate's degree or higher from an accredited college or university, OR * Certification as a Community Healthcare Worker or Certified Recovery Support Specialist Selection Criteria This position requires applicants to complete an interview.
