Throughout the day, we communicate regularly to support patient needs, coordinate care, and ensure continuity. ________________________________________ Key Responsibilities *Provide high-impact, patient-centered care by completing thorough nursing assessments and creating individualized care plans. *Deliver advanced, skilled nursing interventions-including treatments, education, and inpatient-level clinical support-in the patient's home. *Monitor patient status using remote physiologic monitoring and respond promptly to changes in condition. *Coordinate comprehensive care, including diagnostics, medication reconciliation, durable medical equipment (DME), and interdisciplinary services. *Document care efficiently and accurately using Point-of-Care electronic records. *Provide patient and caregiver teaching, support safe transitions, and assist with discharge planning. *Manage your daily schedule with autonomy, aligned with patient needs and program
