The Senior Manager of Hospital Outreach will lead a team of Dialysis Transition Managers (DTMs). These individuals are responsible for conducting outreach to hospitals in an effort to improve the transition of care experience for renal patients transitioning from the hospital to outpatient dialysis. The Senior Manager will serve as a strategic thought partner and operational leader, enabling the evolution of our DTM program. This leader will not only manage and develop a team of high-performing DTMs but will also architect and own high-impact special projects, pilot programs, and strategic initiatives designed to innovate the transition of care experience for renal patients. This leader will drive change by leveraging hospital insights to build scalable strategies, deploying new technologies or processes, and cross-functionally executing on our broader programmatic goals. The Senior Manager is expected to: (1) design and execute complex, multi-stakeholder strategic initiatives, (2) elevate the coaching and development of their team to identify growth opportunities and generate actionable insights, (3) collaborate seamlessly across clinical, operational, and technical business units, and (4) communicate compelling, data-driven recommendations to senior executives.]

If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.
