Care At Home - Who we are: https://youtu.be/FJpe-cbaA7g?si=ula_iBAkPPb7PTPm (Copy and paste into browser) Reporting to the Clinical Programs Manager, the Clinical Coordinator is responsible for working collaboratively with the patient care team to promote the safe and effective use of medications. Patient care is provided over the telephone, in person within an outpatient pharmacy setting, and through pharmacy home visits. The Clinical Coordinator pharmacist performs medication management for patients who are enrolled in value-based clinical programs and pharmacy services. The Clinical Coordinator also takes a leading role in precepting pharmacy residents, students, and interns through the coordination of value-based clinical programs in the outpatient pharmacies. Operational duties include coordinating medication management for patients enrolled in a variety of clinical programs, optimizing medication therapies through clinical assessment and recommendations, and managing medication regimens prescribed by physicians and other authorized prescribers. Minimum Qualifications:
Salary Range: Minimum $45.22/hour - Maximum $81.87/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
