Job Summary

Responsible as a member of the interdisciplinary team to the team manager for the bereavement program to include the provision of supportive services to help meet patient/family social service and emotional needs in bereavement, consistent with Hospice of the Hills policies.

Job Description

Essential Functions:

  • Completes the risk factor assessment from information shared with team members during Hospice admission.
  • Completes the follow-up bereavement assessment with Hospice families within the first month of loss as the documented bereavement plan, which also includes volunteer bereavement plans.
  • Manages the Bereavement Services program, utilizing professional staff and volunteers as appropriate to the service or event.
  • Oversees bereavement follow-up by patient care staff and volunteers.
  • Coordinates assignments of bereavement volunteers with family visits and bereavement events.
  • Coordinates community bereavement education via workshops, teleconferences, inservices and community presentations.
  • Develops educational programs and materials for patients/families, staff and the community on loss, grief, and coping with bereavement.
  • Designs or updates materials for distribution to families eligible for Hospice bereavement services.
  • Participates in marketing and promotion of department and corporation services by being positive role model and ambassador for department and hospital with demonstration of adherence to customer satisfaction standards.
  • All other duties as assigned.

Additional Requirements

Required:
Education - Bachelors degree in Sociology, Psychology, Mental Health, or Health Sciences

Preferred:
Experience - 1+ years of Supervisor Experience; 3+ years of Clinical Experience
Certification - Grief and Loss Certification - Accredited University or accredited training professionals

Physical Requirements:
Light work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
