A Brief Overview The Lead Home Visitor for the Community Asthma Prevention Program will utilize exceptional customer service skills to engage, support and train community health workers in performing their responsibilities. Based on CAPP's training model, they will be responsible for assigning training modules, conducting module reviews, leading and coordinating mock lessons, providing field instruction and supervision and assuring fidelity to CAPP asthma curriculum. They will work directly with CAPP Academy trainees and partners to assure that CHWs meet the partner's needs for in-person and virtual asthma self-management education and environmental mitigation. The Lead Asthma Home Visitor will perform all of the responsibilities of the Home Visitor in the Asthma Program. They also supports leadership in overseeing the Community Health Worker team and their workflow to ensure the effectiveness of the program. They perform routine monthly supervising field visits for Community Health Workers. What you will do Keeps in frequent daily contact with home visitors, is available to make decisions, answer questions and assist with any problems or challenges home visitors face in the field. Utilizes high level engagement techniques to establish rapport with patient families Communicates identified family needs to the larger care team, including the nurse Connects with patient/family (and interpreter services as applicable), in advance of scheduled in-person or virtual appointment to ensure readiness to join encounter Receives and responds to communication from patient/family in order to troubleshoot difficulties in connecting to healthcare or social resources Carry a 60% caseload of home visit program participants Ensure that field work is up-to-date and accurate, following all program protocols Supports patient/family in downloading and logging into virtual encounters Provides troubleshooting support Directs Patient/family to additional resources as needed Maintain ongoing expertise in technologies used by CHOP to engage with patients. Education Qualifications High School Diploma / GED Required Bachelor's Degree Preferred Experience Qualifications At least four (4) years as a community health worker or navigator Required At least three (3) years relevant experience. Required At least one (1) year working in the community with families Preferred Supervision experience, live in service area Preferred Skills and Abilities Ability to travel by car or independence on public transit, taxi, or ride-sharing program Knowledge of local community agencies and organizations. Strong interpersonal skills, communications skills, problem-solving skills, teaching and coaching skills, and community networking skills. Ability to work effectively with a wide range of constituencies in a diverse community.
