Position Summary/Department Summary: We are seeking a compassionate and skilled Clinical Social Worker to join our outpatient, multidisciplinary teams in the Center for Inflammatory Bowel Disease and additional Core GI Clinics including Celiac Disease Center, within the Gastroenterology Department. This is a large, vibrant, fast-paced clinic. This is a 36 hour a week, hybrid position, and involves providing a range of clinical social work services and care coordination to patients and families navigating Crohn's Disease, Ulcerative Colitis, Celiac Disease and other GI diagnoses. This role requires specialized support and comfort in areas including parent coaching, school reintegration and advocacy, chronic illness adjustment, family dynamics and child protection concerns. The ideal candidate will bring strong clinical judgment, and a nuanced understanding of how psychosocial stressors intersect with pediatric medical conditions. Key Responsibilities: Conduct psychosocial screenings and comprehensive assessments to guide care planning Provide brief individual and family support focused on adjustment to chronic illness and family system support Offer longitudinal care coordination and case management, facilitating access to community and hospital-based support, tracking care plans, and addressing psychosocial barriers to follow through Help families navigate the intersection of chronic illness and educational systems and collaborate with schools to implement recommended accommodation and support services Collaborate closely with a multidisciplinary team including physicians, nurses, dieticians, nurse practitioners, psychologists, and other allied health professionals Foster effective team dynamics by facilitating open communication and collaboration among multidisciplinary medical teams Deliver crisis intervention, safety planning, and resource mobilization as needed Address child protection concerns in collaboration with internal and external agencies Contribute to program development and participate in ongoing quality improvement efforts Minimum Qualifications Education: Master's degree in social work required Experience: Previous medical social work experience highly preferred. Exceptional verbal and written communication and organizational skills are essential. Flexibility and ability to work in a fast-paced, dynamic environment is essential. Self-motivation is key! Strong clinical experience in pediatric settings; GI or chronic illness experience a plus. Licensure/Certification: LICSW in the state of MA is required. This schedule is Monday-Friday. Alternating Mondays are at our Needham satellite (free parking!) with the multi-disciplinary team, with other Mondays remote. Friday mornings will be on site in Boston. The additional days will be a combo of Boston/remote. This 36-hour position is eligible for full-time benefits! New hires are eligible for a $4,500 sign on bonus! The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting. Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
