Per Diem - Nurse Practitioner or Physician Assistant

We're seeking a Nurse Practitioner or Physician Assistant to provide personalized care to patients in skilled nursing, assisted living, and senior housing communities. This role offers the opportunity to build relationships with patients, collaborate with an interdisciplinary care team, and improve outcomes for some of our most vulnerable populations.

? $5,000 Sign-On Bonus for external candidates

• Conduct comprehensive patient assessments and physical exams
• Manage chronic conditions and support patients through post-acute rehabilitation
• Collaborate with interdisciplinary care teams to coordinate care
• Lead advanced care planning and goals-of-care discussions
• Educate and support patients and caregivers
• Ensure accurate clinical documentation and quality care delivery

Nurse Practitioners:

• Active ANCC or AANP certification (Family, Adult, Geriatric, or AGPCNP)

Physician Assistants:

• Active NCCPA certification

For All Candidates:

• Active state license (or ability to obtain before start date)
• DEA license or ability to obtain
• Valid driver's license and reliable transportation
• Experience in geriatrics, long-term care, home care, or senior living preferred
