This position requires extensive travel within greater Monmouth & Ocean County New Jersey, as well as the ability to function independently. Care will be provided direct in a patient's home, assisted living facilities, group homes, and/or nursing homes.
Position Highlights:
Primary Responsibilities: A Home Care Physician Assistant or Nurse Practitioners conducts health assessments on enrolled Medicare Advantage members in their personal homes, assisted living facilities, group homes, and/or nursing homes - many of whom are elderly, physically/mentally disabled, and/or homebound. A Home Care practitioner will complete visits on members within a specified geographic area of responsibility. The nature of the Home Care visit can vary and could include performing an annual health assessment, a post discharge visit, or visiting more complex members routine care (i.e., every 4-6 weeks). Practitioners can structure their schedules and visits to coordinate care in the same geographic area(s) daily.

New Jersey Residents Only: The salary range for this role is $109,513 to $147,146 per year. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
