Job Requirements: Provides direct social work services to patients in their homes Works with the interdisciplinary team in integrating social work services into the plan of care Education Master's Degree in Social Work from an accredited School of Social Work Required Licensure/Certification Must have Maryland License in Social Work (LCSW-C) with counseling experience Must obtain current CPR certification Must have valid Maryland Driver's License Work Experience Home Care experience is preferred Experience with management of adult population Clinical experience in counseling and casework for the terminally, chronically, and acutely ill adult population Johns Hopkins Health System and its affiliates are drug-free workplace employers.
