POSITION SUMMARY: Provides quality psychosocial care to our patient's and coordinate care with medical team. Through the coordination of care, the primary purpose is to conduct a thorough assessment of patients' needs, understanding of the medical plan of care, coping abilities, and barriers to care then implement interventions and/or provide resources. Serves as an integral member of the health care team delivering psychosocial care in accordance with JHACH policies and procedures and state and federal regulations. SCHEDULE: Days, including Weekends and call QUALIFICATIONS:
Salary Range: Minimum 26.25 per hour - Maximum 42.00 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
