Summary:
Responsible for coordinating the exchange of pertinent clinical and medical information between the admitting psychiatric facility or prison facility and the current GHI treatment program as well as other relevant involved parties. Short-term casework will also be provided on an as needed basis while long term or alternate treatment is being arranged. Conduct substance abuse treatment with clients in need according to the 12 core functions of substance abuse counseling as defined by the *ICRC.*

This position will have duties and responsibilities in both the Pawtucket (103 Bacon St.) and the Johnston (1443 Hartford Avenue) offices.

Responsibilities:
MAIN DUTIES AND RESPONSIBILITIES:

Assess and monitor consumers while inpatient at a psychiatric facility or incarcerated.

Conducts a mental status exam and risk assessment in preparation for discharge.

Assess the consumers response to inpatient care and discusses the results with the appropriate Hospital and Center staff.

Recommend treatment and disposition based on facts and data gathered in the clinical assessment.

Conduct and document screening assessment of new or readmitted consumers for presentation and review at ES disposition meetings.

Conduct record reviews to establish behavior patterns of long term inpatient consumers and recommends alternative placements when appropriate.

Coordinate and monitors treatment services for all assigned cases.

Provide hospital staff with comprehensive history of patient including past and hospitalizations and past and current treatment plan.

Communicate to hospital staff the recommendations of the GHI treatment program and psychiatrist and reports on the response of the hospital staff to the Gateway treatment.

Participate in treatment plan meetings and difficult case conferences.

Obtain release of information from patients to gather past treatment history in an effort to conduct an accurate assessment and diagnosis and to facilitate the coordination of appointments and/or referrals.

Coordinates and arranges treatment and services with other agencies and appropriate resources.

Coordinates discharge plans and services for clients returning to the community and ensures that they are carried out in a timely manner as identified in their plan of treatment.

Attend and acts a witness at Civil Court on behalf of the consumer and/or treatment team. Facilitates information and preparation for Civil Court as needed.

Coordinate all applications for long term care.

Provides short term casework and support to the consumer and their family while longer term services are secured.

Assists clients who do not meet criteria for admission to Gateway for necessary referrals.

Coordinates services for clients returning to the community including intake follow up psychiatric appointments casework services/appointments transportation and referrals for psychiatric rehabilitation.

Identifies recommends and consults regarding referral alternatives for consumers who do not meet criteria for admission to Gateway.

Ensures responsibly is assigned to all aspects of the discharge plan.

Provides all necessary information to staff upon discharge.

Ensures prison medical record is available as well as probation or parole status.

Obtains proper insurance authorizations from care managers as needed in securing treatment appointments upon discharge.

Ensures documentation compliance with licensure contract and GHI guidelines.

Obtain proper insurance authorization as appropriate for discharge.

Ensure correct information transfer from hospital to Gateway.

Documents all inpatient admissions according to hospital tracking procedure.

Represent GHI at community agency meetings and ensure flow of information.

Actively discusses and monitors the status of Hospital admissions with hospital staff.

Works cooperatively with care managers of insurance providers.

Provides psychoeducational services to clients and their families/significant others based on modality frequency participant and location specifications prescribed in the individual treatment plan. Services will include provision of information regarding mental illness alcohol and substance abuse effective parenting behavior management as well as support groups for caregivers of individuals with behavioral health issues.

Educates consumers and their families on recommended program.

Educates consumers and their families on the how to access the GHI Emergency Services department in the event there is the need.

Provide education to outside agencies as necessary regarding GHI services.

Acts as backup liaison to Gateway Emergency Services and ES screenings teams at Pawtucket and Johnston sites.

Schedules duties in coordination with other ES clinicians and modifies work schedule as needed to ensure that the department is responsive to the consumer*s needs.

Attends all necessary meetings.

Perform data collection as needed.

Performs other duties as assigned.

QUALIFICATION REQUIREMENTS:

Associate*s degree required. Bachelor*s degree preferred.

Two years direct experience treating persons with severe mental illness.

JOB KNOWLEDGE SKILL AND ABILITY:

Read analyze interpret professional journals policy and procedures and governmental regulations.

Clearly and cogently document clinical information in accordance with Gateway policy and procedure.

Write reports and general correspondence.

Effectively present information and respond to questions from professionals managers clients family members and the general public.

Solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.

Interpret a variety of instructions furnished in written oral and diagram or schedule form.

Analyze client and system information evaluate staff identify problems establish facts draw valid conclusions develop options and strategies for resolving problems monitor implementation and evaluate the problem solving process for effectiveness.

Ability to successfully complete all required training and certifications

Oral and written communication skills including speech grammar spelling and legibility are at sufficient level to ensure effective and appropriate communication.

Ability to add subtract multiply and divide in all unites of measure using whole numbers common fractions and decimals either manually or by use of a calculator.
