Physical Therapist PRN

Job Description:

Baptist Health is looking for a Physical Therapist to work PRN for our Home Health Agency in Lexington, KY.

• Performs initial and ongoing assessments.
• Establishes and updates the plan of care in concert with the physician.
• Communicates the patient's needs/changes in condition to the physician and team members.
• Accurately documents all care and coordination of care within the specified time according to system and agency policies, regulatory and payor requirements.
• Incorporates all regulatory and accreditation guidelines in both planning and providing care.
• Provides clinical supervision for PTA's. and home health aides
• Responsible to meet minimum productivity.
• Demonstrates competency in physical therapy skills.
• Participates in performance improvement activities to maximize patient outcomes and the patient experience.
• Attends agency meeting as required and completes all mandatory education according to policy.
• Other duties as assigned.

Minimum Education, Training, and Experience Required

• Advanced Degree PT
• Professional license PT
• Valid Driver's License
• BLS Healthcare Provider
• One year of experience as a Physical Therapist
• Home Health experience preferred.

Physical Therapist License, BLS, Home Care.
