SUMMARY

The Medication Use & Drug Information Coordinator is responsible for oversight and coordination of systematic additions, removals and standardization of medications across the health system, in addition to providing support for the development and implementation of medication use protocols. This position reports to the Director of Pharmacy Techonology, Informatics, and Business Operations.

- Facilitates and oversees all additions, removals, and standardization of medications across the health system.
- Coordinates systematic medication changes with EMR, automation, operational, and clinical teams.
- Leads departmental and organizational medication-related quality improvement initiatives.
- Leads educational efforts associated with systematic medication-related changes.
- Serves as the primary liaison between clinical and operational groups and EMR architecture groups to coordinate and validate EMR modifications associated with medication changes.

- Precepts pharmacy students and pharmacy residents.
- Triages and answers drug information questions from providers, nursing, and pharmacy staff.

- Provision of patient-centered, evidence-based pharmaceutical care.
- Evaluate and monitor drug regimens for appropriateness.
- Review and Interpret physician's orders and recommend changes and interventions when needed.
- Provide patient and nursing education when requested.

Other Duties as Assigned

QUALIFICATIONS

- PharmD from an accredited school of pharmacy

- Two (2) or more years job related experience including medication use oversight, formulary management, and collaboration with EMR architecture teams.

- Completion of a PGY-1 pharmacy residency or equitable experience (3 years) in a health system of informatics role.

- Licensed Pharmacist in the State of GA
