Fallon Health's Summit ElderCare is a Program of All-Inclusive Care for the Elderly-PACE for short. PACE, an alternative to nursing home care, is a program that helps people 55 and older continue living safely at home. Position Overview:

Most of our staff Physicians spend the bulk of their days at our PACE site and interact with our participants in that location. In this very unique SNFIST opportunity, we are looking for someone who enjoys the independence being more out and about in the community we service and will interact with a participant panel at our SNF type facilities such as Senior Living and Nursing Locations, Long Term Care, Rehab Centers. In the role of an MD SNFIST your schedule and activities will include:

    Providing medical oversight of patients in the facilities
  • Coordinating with Summit Eldercare PCP
  • Rounding and completing required visits as per facility, Summit Eldercare and regulatory protocols and requirements
  • Participating in daily morning IDTs
  • Previous experience working in rehab and skilled nursing facilities preferred.
  • The team uses a collaborative approach to care planning and is called the Interdisciplinary Team (IDT). The IDT is comprised of Providers (MD/DO and NP/PA), Nurses, Social Workers, Physical and Occupational Therapists, Dieticians, Health Aides, Recreation staff, Home Care and Transportation Coordinators and Site Administrator. This approach to care is the gold standard for complex and frail older adults who wish to avoid nursing home placement.

Summit ElderCare is the only program of its kind in central Massachusetts. Summit ElderCare started as part of a PACE demonstration project, and is one of Eight PACE programs in the State. There are more than 118 PACE programs nationwide. And, although Fallon Community Health Plan sponsors Summit ElderCare, a participant does not have to be an FCHP member to enroll.

Summit ElderCare is committed to maximizing the dignity and respect of older adults and to enable elders to remain in their homes and in their communities as long as medically and socially feasible. Summit ElderCare enhances their quality of life and their autonomy. Summit ElderCare preserves and supports the elderly adult's family as an integral part of this mission.

Our history

Established in 1995 as an alternative to nursing home placement, Summit ElderCare is focused on caring for older adults in an environment that helps enhance their quality of life and autonomy by helping them to stay in their homes and communities.

Recognizing a community need for more comprehensive services, care and support for frail elders who want to remain independent, and their caregivers, FCHP founded the Elder Service Plan, now known as Summit ElderCare. It started as part of a statewide project in Massachusetts under the leadership of the East Boston Neighborhood Health Center's PACE program. The program began with just eight participants. Since then, hundreds of participants and their caregivers have received services from the program.

Today, Summit ElderCare is one of six PACE programs in Massachusetts and more than 90 programs nationwide. This Program of All-inclusive Care for the Elderly (PACE) first became available to residents of Worcester County, the city of Marlborough and the town of Hudson in 1995. The program was provided permanent provider status by the Centers for Medicare & Medicaid Services in September 2002.

Qualifications

Qualifications:

Education: Medical degree, Geriatric specialty strongly desired

License: A doctor of medicine or osteopathy legally authorized to practice medicine or surgery in the Commonwealth of Massachusetts. Current DEA certificate.

Certification: Board certification Internal Medicine, Geriatrics or Family Practice preferred

Experience: Two or more years working with a frail or elderly population. Previous managment experience desirable.
