I. Major Responsibilities:
1. Provides patient care according to standards of practice and departmental policy and procedure.
2. Provides assistance in patient education and documents appropriately according to policy and procedure.
3. Achieves continuity of patient care through planning and exchange of information with other health care professionals.
4. Administers medications safely and in accordance with professional standards of practice and departmental policies and procedures within the scope of practice for the LPN.
5. Makes recommendations for changes to the plan of care to the appropriate RN or other team member as the patient condition warrants.
6. Delegates patient care activities to appropriate personnel in accordance with professional standards of practice and departmental policy and procedure and within the scope of practice.
7. Meets productivity guidelines (7 visits per 8-hour day) according to departmental policy and procedures.
8. Supports performance improvement initiatives through active participation in PI processes, projects, and teams.
9. Creates and maintains documentation, which is consistently complete, accurate, timely and legible in accordance with departmental policies and procedures.
10. Maintains a safe environment for our patients through consistent compliance with policies and procedures.
11. Possesses and applies the skills and knowledge necessary to provide care to patients and customers throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.

Standard Staffing Level Responsibilities:
1. Complies with established departmental policies, procedures, and objectives.
2. Attends variety of meetings, conferences, seminars as required or directed.
3. Demonstrates use of Quality Improvement in daily operations.
4. Complies with all health and safety regulations and requirements.
5. Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
6. Maintains, regular, reliable, and predictable attendance.
7. Performs other similar and related duties as required or directed.

II. Position Qualifications:

License/Certification/Education:
Required:

1. Current licensure as a Licensed Practical Nurse in the Commonwealth of Massachusetts.
2. Successful completion of an accredited Licensed Practical Nurse Program.
3. Valid US-issued driver's license.
4. Registered, inspected, and insured automobile for work related purposes.

Preferred:
1. BLS certification.
2. CPR certification.

Experience/Skills:
Required:

1. Minimum two (2) years of Medical-Surgical nursing experience in direct patient care.
2. Ability to perform detailed assignments with minimal supervision and work under pressure.
3. Knowledge of the nursing process for the delivery and management of patient care.
4. Ability to create electronic documentation, which is accurate, intelligible, and concise.
5. Ability to complete and maintain all required licensed practical nurse's competencies.
6. Ability to communicate proactively and clearly with patients, caregivers, and medical staff.
7. Ability to read, write and speak clearly in English.

Preferred:
1.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
