Bronx Health Center and Foster Care Services (910 East 172nd St)

$50,000 - $65,000

*Sign-On Bonus Eligible*

Essential Duties:



  • Understand and promote the organization's Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
  • Work with a dynamic team of professionals including nurses and social workers in our foster care program for children with special medical needs.
  • Provide intensive casework services and fieldwork for children and their birth and foster families with the goal of providing permanency as quickly as possible.
  • With medical staff, team-train foster parents in specialized child care skills.
  • Conduct intensive outreach to connect children and families with agency and community treatment resources.
  • Maintain written case records.
  • Work in partnership with RN assigned to cases; Collaboration includes conducting home visits whenever possible.
  • Perform other duties as assigned.



Minimum Qualifications:



  • Bachelor's Degree in Social Work or related field required, Master's degree preferred.
  • Foster care experience preferred;
  • Bilingual (Spanish/English) preferred.

  • Excellent verbal, writing, and organizational skills;
  • Flexibility, and ability to work under pressure and meet deadlines;
  • Computer literacy;
