DESCRIPTION The Wound Care Clinician will be accountable for evaluating and treating wound and ostomy patients across the continuum of care. The wound care clinician will collaborate with frontline staff to enhance clinical outcomes, throughput, quality, and customer service. Ensure the standard care of the department and service line is executed. Collaborate and communicate with nursing, medical/surgical teams, and other healthcare members to ensure patient safety and the patient's needs are met. The direction from leadership will drive the actions carried out by the wound care clinician to ensure departmental quality improvement initiatives and the hospital system goals are met. This position will report to the Wound Care Department Director JOB RESPONSIBILITIES Performs consultation, evaluation, and treatment of patients within the care of the Grady Health System. Executes and facilitates excellent customer service to the departmental partners and patients. Collaborate with nursing teams and other healthcare teams to ensure patient satisfaction. Facilitates improved communication amongst the department and all healthcare partners. Provide expert advice or consultation to staff & and other interdisciplinary clinicians. Participates in interdisciplinary team rounds and communicates effectively. Coaches staff in the delivery of care and assuring the care is best practices. Performs detailed and comprehensive evaluation of wound/ostomy care needs of patients. Provides expertise recommendations to patients, families and staff. Applies approved skin substitution applications and manages complex negative pressure. Communicates patient barriers or quality concerns to leaders/staff and medical teams. Participated in daily huddles to report safety concerns, and escalate any barriers to leaders. Demonstrate Grady Values and HRO principles. Maintains departmental standards and productivity requirements. Knowledgeable of internal and external resources to ensure patient safety and departmental throughput. EDUCATION Required: Bachelor's Degree in Nursing, Physical Therapy, or Occupational Therapy Preferred: Master's Degree in Nursing, Physical Therapy, or Occupational Therapy EXPERIENCE Required: Two (2) or more years job related experience in Nursing, Physical Therapy, or Occupational Therapy. Preferred: Four (4) or more years job related experience in Nursing, Physical Therapy, or Occupational Therapy. SKILLS/CERTS/LICENSES Required: Basic Life Support (BLS) Registered Nurse in State of Georgia (for Nursing applicants only) Licensed Occupational Therapist in the State of Georgia (for OT applicants only) Licensed Physical Therapist in the State of Georgia (for PT applicants only) Preferred: Certified in Wound Care |
